Financial and Deposit Policy

• Complete payment is due at the time of service. 
• Patients with dental insurance are expected to pay their estimated copay and deductible at the time of service.
• We accept cash, checks, Apple/Google Pay, CareCredit, and all major credit cards (Visa, Mastercard, American Express, and Discover). 
• All treatment will require a 50% deposit at the time of scheduling.
• Payment in full may be required for extensive treatment that requires lab fees and/or longer procedure time.
• If the patient’s insurance company has not paid the claim in full, the remaining balance is the patient’s responsibility.
• Any balance carried more than 45 days will be assessed a $50 delinquency fee and sent to collections.

Appointment/Cancellation Policy

• Solid Rock Dental asks that you cancel/reschedule any appointment at least 24 hours in advance.
• Monday appointments need to be confirmed, changed, or canceled by the previous Friday 12 pm.
• Any appointment not confirmed 24 hours BEFOREHAND may be canceled. 
• Missed or inappropriately canceled appointments will be assessed a $50 fee.
• Solid Rock Dental reserves the right to dismiss patients due to habitually missed appointments.
• All minor patients must be accompanied by an adult parent or legal guardian. 
• It is the patient’s responsibility to provide accurate and up-to-date insurance information.
