Membership Plan

Read Our Terms and Conditions

1. Term: Membership is good for one year from the date of enrollment and nonrefundable. Your membership benefits expire one year from your enrollment date, so any unused benefits will be forfeited. 
2. Appointment: Members must set up their two appointments at the time of enrollment. Please notify our office at least 24 hours in advance for any appointment changes.
3. Discount: To receive a 10% discount on dental treatment, the payment is due on the date of service.
4. Qualification: Membership is only for patients without dental insurance. 
5. Services Provider: Plan benefits apply to treatments provided by Solid Rock Dental. 
6. Fee: Membership fee is due at time of enrollment. 

Payment Method: Solid Rock Dental accepts cash, check, debit cards, and credit cards as payment methods.
